Armstrong County CBOC

Located on the grounds of Armstrong County Hospital in Kittanning, this clinic provides primary care, physical exams, weight managment, smoking cessation counseling, and routine laboratory.  Outpatient behavioral health services including individual therapy and medication management are also available at the clinic and limited routine radiology is available next door in the main hospital.  The clinic is open 8:00 a.m. - 4:30 p.m. Monday through Friday.

Directions:

Route 422 to the West Hills exit

From the East follow the exit around and bear right.  At the next set of crossroads, turn left and you will be on Nolte Drive (the State Police Barracks will be on your right).  Follow the road to the back of the hospital and the clinic is located in Suite 120.

From the West exit and go to the 4-way intersection.  Go straight on Nolte Drive, the State Police Barracks will be on your right.  Follow the road to the back of the hospital and the clinic is located in Suite 120.

Clarion County CBOC  

Located in southwestern Clarion County, PA just outside of historic Foxburg, this clinic provides primary care, physical exams, weight management, smoking cessation counseling, and routine laboratory.  Outpatient behavioral health services including individual therapy and medication management are also available.  The clinic is open 8:00 a.m. - 4:30 p.m. Monday through Friday, except they are closed between 12 Noon and 1 p.m. daily.

Directions:

From Interstate 80 take Exit 45, Emlenton/St. Petersburg.  Take route 478 South to St. Petersburg. Turn right on to Route 58 South.  AC Valley Medical Center will be located on your right across from the AC Valley High School.  Enter from the upper parking lot entrance.

From Route 268 follow to Route 58 North.  Cross bridge over the Allegheny River at Foxburg.  Follow Route 58 North past the Foxburg Country Club.  The clinic will be located on your left across from the AC Valley High School.  Enter from the upper parking lot entrance.

Lawrence County CBOC

Opening October 2007! Conveniently located on Business Route 422 east of New Castle, PA, the clnic will provide primary care, physical exams, weight management, smoking cessation counseling, and route laboratory.  Outpatient behavioral health services are planned including individual and group therapy and medication management.  Referrals to a psychiatrist in Butler may also be provided through secure videoconferencing from the clinic.  The clnic will be open 8:00 a.m. - 4:30 p.m. Monday through Friday.  Limited services are currently available in New Castle and Ellwood City.  Those clinics will be closed and patients will automatically be

transferred to the new location.  New patients interested in enrolling at the Lawrence County VA Outpatient Clinic may call (800)362-8262, extension 5011 toll free or (724) 477-5011.  If you already have a VA primary care provider and would like to transfer to the new clinic, please contact your primary care health technician.

From Route 60:  Take Route 422 East to the Butler Exit onto Business Route 422 West.  Clinic will be located on your left in the Ridgewood Professional Centre.

From Interstate 79:  Take Exit 99, New Castle Route 422 West approximately 11 miles.  Follow Business Route 422 to the Clinic which will be located on your left in the Ridgewood Professional Centre.

A Home Based Primary Care Team located in this clinic provides home care services to eligible veterans within a 30 mile radius.  Coming in 2008! Routine radiology services will be available at the clinic.  The clinic is open 8:00 a.m. - 4:30 p.m. Monday through Friday.

From the South:  Take route 60 North to route 18 North.  Take route 18 North approximately 3 miles to E. State Street (Route 62).  Turn left onto E. State Street.  Take E.
